چکیده
اختلال عملکرد جنسی یکی از عوارض شایع در بیماران مبتلا به سرطان پستان و ژنتیال است که تأثیر قابلتوجهی بر کیفیت زندگی، رضایت زناشویی و تبعیت از درمان دارد. یک بررسی مقطعی اخیر بر روی ۲۲۳ زن مبتلا به سرطانهای پستان و تولیدمثلی در سبزوار (۱۴۰۲) نشان داد بیش از ۹۳% آنان دچار اختلال عملکرد جنسی هستند. در این سند سیاستی مستندات موجود در ایران و سایر نقاط جهان پس از مرور با یکدیگر تلفیق شد تا توصیههای عملیاتی ارائه شود. سیاستهای پیشنهادی بهطور کل شامل ادغام برنامههای آموزشی و غربالگری در خدمات انکولوژی، ارجاع، حمایتهای بیمهای و مالی جهت دریافت مشاوره سلامت جنسی و درمانهای مربوطه در حین درمان و دوران بعد از درمان است. اجرای این راهحلها مستلزم توجه به ملاحظات حیاتی است. غلبه بر موانع فرهنگی با طراحی برنامههای آموزشی حساس به بافت اجتماعی ضروری است. از جنبه سازمانی، تأمین بودجه پایدار و آموزش نیروی انسانی متخصص پیشنیاز موفقیت است. درنهایت، حمایت سیاستی از طریق گنجاندن خدمات در راهنماهای بالینی ملی و ایجاد الزام قانونی، پایداری این اقدامات را تضمین میکند.
اجرای این سیاستها نهتنها کیفیت زندگی بیماران را بهطور اساسی بهبود میبخشد، بلکه از طریق افزایش تبعیت از درمان، در بلندمدت به صرفهجویی در هزینههای نظام سلامت، ارتقاء باروری و کمک به جوانی جمعیت منجر خواهد شد.

Abstract
Sexual dysfunction is a highly prevalent yet often neglected consequence of breast and gynecologic cancers, significantly affecting patients’ quality of life, marital satisfaction, and treatment adherence. A recent cross-sectional study conducted in Sabzevar (2023) involving 223 women with breast and gynecologic cancers revealed that over 93% experienced sexual dysfunction. This policy summary integrates national and international guidelines to formulate practical recommendations. The proposed policy framework recommends integrating educational and screening programs into oncology services, facilitating referrals, and ensuring financial coverage for sexual healthcare during and after treatment. Implementation requires a multi-faceted approach: designing culturally adapted programs to overcome barriers, securing sustainable funding and specialized training from an organizational standpoint, and support through national clinical guidelines and legislation to ensure lasting change.
