چکیده

زمینه و هدف: هدف این مطالعه، ارزیابی اثربخشی آمبولیزاسیون شریان رحمی (UAE) در کنترل خونریزی غیرطبیعی رحم (AUB) ناشی از فیبروم و بررسی تأثیر آن بر حفظ سیکل قاعدگی، رضایت بیماران و پیامدهای آن بود.
مواد و روش­ها: این مطالعه گزارش موردی بر روی ۱۷ زن ۲۱ تا ۴۲ ساله انجام شد که طی سال‌های ۱۳۹۸ تا ۱۴۰۱ در بیمارستان شهدای تجریش تحت UAE قرار گرفتند. اطلاعات جمعیت شناختی، الگوی قاعدگی، تعداد فرزندان، مدت بستری، عوارض و وضعیت باروری جمع‌آوری شد. بیماران در یک دوره پیگیری 3 تا 12 ماه به‌صورت ویزیت حضوری یا تماس تلفنی ارزیابی شدند. تحلیل داده‌ها با SPSS نسخه ۲۵ و با استفاده از آزمون‌های توصیفی انجام شد.
یافته­ها: در تمام موارد (۱۰۰٪)، علت AUB وجود میوما بود. منوراژی شایع‌ترین الگوی خونریزی (۴۷.۱٪) بود، درحالی‌که قاعدگی طبیعی (۴۱.۲٪)، یائسگی (۵.۹٪) و متروراژی (۵.۹٪) نیز گزارش شد. بیشترین تعداد فرزندان مربوط به دو فرزند (۵۴.۲٪) بود. ۶۴.۷٪ بیماران ظرف ۲۴ ساعت مرخص شدند و ۳۵.۳٪ حداکثر تا ۴۸ ساعت بستری بودند. تب شایع‌ترین عارضه پس از UAE (۴۱.۲٪) بود. طی پس از عمل، همه بیماران از سیکل قاعدگی برخوردار بودند.
نتیجه‌گیری: UAE روشی ایمن و مؤثر برای درمان AUB ناشی از میوماست که در پیگیری ۳ تا ۱۲ ماهه، چرخه قاعدگی در تمام بیماران به‌صورت نرمال بود؛ بنابراین، می‌توان آن را جایگزینی مناسب برای جراحی در زنان دانست. به دلیل حجم نمونه کم و طراحی گذشته‌نگر، انجام مطالعات کوهورت دو گروهه برای مقایسه اثربخشی UAE با سایر روش‌ها توصیه می‌شود.

Evaluation of the Indications, Effectiveness, and Outcomes of Uterine Artery Embolization in Patients with Abnormal Uterine Bleeding or Postpartum Hemorrhage at Shahid Tajrish Hospital from 2019 to 2022

Introduction: This study aimed to evaluate the effectiveness of uterine artery embolization (UAE) in controlling fibroid-related abnormal uterine bleeding (AUB) and to assess its impact on fertility preservation and patient satisfaction.
Materials and Methods: This retrospective studyincluded 17 women aged 21-42 years who underwent UAE between 2019 and 2022 at Shohada Tajrish Hospital. Data on demographic, menstrual patterns, number of children, length of hospital stay, complications, and post-treatment fertility status were collected from medical records. Patients were followed for 3 to 12 months through in-person visits or phone calls. Data were analyzed with descriptive and inferential statistical tests using SPSS version 25. (P<0.05).
Results: Uterine fibroids were the cause of AUB (100%). The most common bleeding pattern was menorrhagia (47.1%), followed by normal menstruation (41.2%), menopause (5.9%), and metrorrhagia (5.9%). Most patients (54.2%) had two children. Most patients (64.7%) were discharged within 24 hours, while 35.3% were discharged within 48 hours. Fever was the most frequent post-procedure complication (41.2%). All patients preserved their menstruation. All patients reported preserved menstrual function at follow-up.
Conclusion: Uterine artery embolization (UAE) proved to be a safe and effective treatment for fibroid-related abnormal uterine bleeding (AUB) in this study, with preservation of menstrual function observed throughout the 3–12-month follow-up period. These findings suggest that UAE may be a suitable uterus-preserving alternative to surgery. However, these results should be interpreted with caution due to the study's retrospective design and small sample size. Larger prospective studies are warranted to confirm these outcomes.
