چکیده
زمینه و هدف: نارسایی مزمن کلیه به نقص غیر قابل بازگشت عملکرد کلیه ها گفته می شود. این اختلال همه ابعاد زندگی فرد و جامعه را تحت تاثیر قرار میدهد. هدف این مطالعه بررسی اتیولوژی کودکان مبتلابه بیماری مزمن کلیه مراجعهکننده به مراکز منتخب نفرولوژی در شهرستان مشهد طی سال های 1383 تا 1402 است.
مواد و روشها: در مطالعه مقطعی حاضر همه کودکان سه ماه تا بیست سال مراجعه کننده از سال 1383 لغایت 1402 با تشخیص بیماری مزمن کلیه، موردمطالعه قرار گرفتند. اطلاعات جمعیت شناختی، بالینی و پاراکلینیکی این کودکان جمعآوری و توسط پژوهشگران در سامانه PCKDR.ir ثبت شد. اطلاعات با استفاده و کمک نرمافزار IBM-SPSS V20 و آزمونهای آماری دقیق فیشر و کای دو تجزیهوتحلیل شد.
یافتهها: تعداد 189 کودک مبتلا محدوده سنی 3 ماه تا 20 سال وارد مطالعه شدند. بیشترین شیوع مربوط به جنس پسر (62/56 درصد) بود و 38/43 درصد، جنسیت دختر داشتند. میانگین سنی 43/4 ±10/9 سال گزارش شد. درمجموع، اغلب بیماران (71/85 درصد) در مراحل اولیه بیماری مزمن کلیه (مراحل 1 و 2) قرار داشتند. همچنین، شایعترین اتیولوژی به ترتیب شامل عفونت ادراری ) 75/22 درصد)، سندرم نفروتیک (4/16 درصد) و سنگ کلیه (17/12 درصد) بود.
نتیجهگیری: نتایج این مطالعه نشان داد که شایعترین اتیولوژی های بیماری مزمن کلیوی جزو موارد قابلپیشگیری و دارای درمان ساده هستند که با تشخیص بهموقع و پیگیری میتوان از پیشرفت مراحل بیماری و آسیب دائمی کلیهها جلوگیری نمود.

The Etiologic Spectrum of Chronic Kidney Disease in Children: A Retrospective Cohort Study in Mashhad, Iran

Introduction: Chronic kidney disease (CKD) is an irreversible impairment of kidney function that significantly impacts both individual and societal well-being. This study aims to investigate the etiology of chronic kidney disease in children referred to selected nephrology centers in Mashhad from 2004 to 2024.
Materials and Methods: This cross-sectional study included all children aged 3 months to 20 years who were referred to the nephrology clinic with a diagnosis of chronic kidney disease between 2004 and 2024. Demographic, clinical, and laboratory data were recorded in the PCKDR.ir system. The data were analyzed using IBM SPSS Statistics version 20, employing statistical tests such as Fisher's exact test and Chi-square.
Results: A total of 189 children, aged 3 months to 20 years, were included in the study, with a higher prevalence in boys (56.62%) compared to girls (43.38%). The mean age of the participants was 9.10 ± 4.43 years. Most patients (85.71%) were in the early stages of chronic kidney disease (stages 1 and 2). The most common etiologies were urinary tract infection (22.75%), nephrotic syndrome (16.4%), and kidney stones (12.17%).
Conclusion: The findings indicate that the most prevalent etiologies of chronic kidney disease are preventable and treatable. With timely diagnosis and appropriate follow-up, the progression of the disease and permanent kidney damage can be mitigated.
