چکیده
زمینه و هدف: درد نوروپاتیک، یک بیماری پیچیده و ناتوانکننده است که از آسیب یا اختلال عملکرد سیستم عصبی پیکری ناشی میشود و با اثرات فیزیکی، روانی و اقتصادی قابلتوجهی همراه است. از آنجا که کارکنان مراقبتهای بهداشتی به دلیل نیازهای فیزیکی شغل خود آسیبپذیر هستند، بنابراین مطالعه حاضر باهدف بررسی شیوع درد مزمن نوروپاتیک و عوامل مرتبط با آن در بین کارکنان بیمارستان واسعی سبزوار انجام شد.
مواد و روشها: مطالعه مقطعی حاضر شامل 202 نفر از کارکنان بیمارستان بود که به روش نمونهگیری تصادفی انتخاب شدند. دادهها از طریق پرسشنامه جمعیتشناختی و پرسشنامه DN4 جمعآوری شد. از آزمونهای t، مجذور کای و ضریب همبستگی برای تجزیهوتحلیل آماری استفاده شد (P<0.05).
یافتهها: از بین شرکتکنندگان، 45 نفر (3/22%) دارای امتیاز DN4≥4 بودند که نشاندهنده شیوع بالای درد نوروپاتیک بود. کمردرد شایعترین درد مزمن گزارششده (30%)، پسازآن درد گردن (20%) و درد پا (20%) بود. ارتباط معنیداری بین نمرات DN4≥4، سن (001/0>P)، قد (001/0>P) و سابقه کار (001/0>P) مشاهده شد. نمرات DN4≥4 با درد مزمن در ناحیه کمر (001/0>P)، شانهها (001/0>P) و پاها (012/0=P) ارتباط معنیداری داشت. هیچ ارتباط معنیداری با وضعیت تأهل، ساعات کار، نوع شغل یا سطح تحصیلات یافت نشد.
نتیجهگیری: درد مزمن نوروپاتیک بر بخش قابلتوجهی از کارکنان مراقبتهای بهداشتی، بهویژه در مناطقی مانند کمر، گردن و شانهها تأثیر میگذارد. مداخلات هدفمند، ازجمله تمرینات ارگونومیک و غربالگریهای منظم برای بهبود رفاه و بهرهوری ضروری است. تحقیقات بیشتری برای کشف استراتژیهای مدیریت بلندمدت درد نوروپاتیک در محیطهای مراقبتهای بهداشتی موردنیاز است.

Prevalence and Determinants of Chronic Neuropathic Pain among Healthcare Workers: A Cross-Sectional Study from Vasei Hospital, Sabzevar

Introduction: Neuropathic pain is a complex and debilitating condition resulting from damage or dysfunction of the peripheral nervous system, associated with significant physical, psychological, and economic repercussions. Healthcare workers are particularly vulnerable due to the physical demands of their roles. This study aimed to investigate the prevalence of chronic neuropathic pain and its associated factors among employees at Vasei Hospital in Sabzevar.
Materials and Methods: This cross-sectional study included 202 hospital employees selected through random sampling. The presence of neuropathic pain was assessed using the DN4 (Douleur Neuropathique 4) questionnaire. Statistical analyses, including t-tests, chi-square tests, and correlation coefficients, were performed (P<0.05).
Results: Among the participants, 45 (22.3%) had a DN4 score ≥4, indicating a high prevalence of neuropathic pain. The most commonly reported chronic pain was back pain (30%), followed by neck pain (20%) and leg pain (20%). Significant associations were observed between DN4 scores ≥4 and age (P<0.001), height (P<0.001), and work experience (P<0.001). DN4 scores ≥4 were also significantly associated with chronic pain in the back (P<0.001), shoulders (P<0.001), and legs (P=0.012). No significant associations were found with marital status, working hours, job type, or education level.
Conclusion: Chronic neuropathic pain significantly affects healthcare workers, particularly in the back, neck, and shoulders. Targeted interventions, including ergonomic exercises and regular screenings, are essential for improving well-being and productivity. Further research is necessary to explore strategies for the long-term management of neuropathic pain in healthcare settings.
