چکیده

مقدمه: ختنه از شایع‌ترین اعمال جراحی در کودکان است و می‌تواند با عوارضی همراه باشد. این مطالعه با هدف تعیین فراوانی، الگو و عوامل مرتبط با عوارض پس از ختنه در کودکان مراجعه‌کننده به بیمارستان شهید مطهری ارومیه انجام شد.

مواد و روش‌ها: این مطالعه توصیفی-تحلیلی گذشته‌نگر بر روی ۳۰۰ کودک پسر که طی نیمه نخست سال ۱۴۰۴ با روش‌های حلقه (Plastibell) یا Sleeve ختنه و حداقل ۶ ماه پیگیری شدند، انجام شد. عوارض براساس زمان بروز به زودرس و دیررس و براساس طبقه‌بندی Clavien-Dindo درجه‌بندی شدند. دو روش جراحی با تطبیق بر اساس امتیاز تمایل مقایسه و عوامل مرتبط با بروز عارضه با رگرسیون لجستیک چندمتغیره بررسی شد.

یافته‌ها: در مجموع، ۲۹ کودک (۷/۹ درصد) دچار حداقل یک عارضه پس از عمل شدند. از این تعداد، ۱۸ مورد (۶ درصد) در ۳۰ روز نخست و ۱۱ مورد (۳/۷ درصد) پس از ۳۰ روز رخ دادند. عوارض ساختاری شایع‌ترین نوع عارضه بودند. بیشتر عوارض در درجات I تا IIIb قرار گرفتند و موردی در درجات IV و V مشاهده نشد. پس از تطبیق، تفاوت معنی‌داری میان دو روش ختنه با حلقه و Sleeve از نظر بروز کلی، زودرس، دیررس و شدید وجود نداشت. سن و شاخص توده بدنی نیز پیش‌بینی‌کننده معنی‌داری برای بروز عارضه نبودند.

نتیجه‌گیری: عوارض پس از ختنه در این مرکز نسبتاً کم و عمدتاً خفیف تا متوسط بودند. با توجه به سهم قابل‌توجه عوارض دیررس، پیگیری حداقل ۶ ماهه توصیه می‌شود و انتخاب روش جراحی می‌تواند بر اساس شرایط بالینی کودک و تجربه جراح انجام گیرد.

Post-Circumcision Complications and Associated Factors in Children Attending Motahari Hospital, Urmia

Background: Circumcision is one of the most common surgical procedures in children and may be associated with early and late complications. This study aimed to determine the incidence, pattern, and associated factors of post-circumcision complications in children attending Motahari Hospital, Urmia, Iran.

Methods: This retrospective observational study included 300 boys who underwent circumcision using either the Plastibell or sleeve technique during the first half of 2025 and were followed for at least 6 months. Complications were classified as early or late according to the time of onset and graded using the Clavien-Dindo system. Propensity score matching was used to compare the two surgical techniques, and multivariable logistic regression was performed to assess factors associated with complications.

Results: Overall, 29 children (7.9%) experienced at least one postoperative complication. Of these, 18 events (6.0%) occurred within the first 30 days and 11 events (3.7%) occurred after 30 days. Structural complications were the most common type. Most complications were graded I to IIIb, and no grade IV or V complications were recorded. After matching, no significant differences were found between the Plastibell and sleeve techniques regarding overall, early, late, or severe complications. Age and body mass index were not significant predictors of complications.

Conclusion: Post-circumcision complications in this center were infrequent and mainly mild to moderate. Given the notable proportion of late complications, follow-up for at least 6 months is recommended, and the choice of surgical technique may be guided by the child’s clinical condition and surgeon experience.
