چکیده

زمینه و هدف: مطالعه حاضر با هدف مقایسه تأثیر دو روش طب فشاری و زنبورک ویبره بر موفقیت اولین تلاش در دسترسی وریدی و کاهش درد ناشی از آن در کودکان 6-3 سال انجام شد.
مواد و روش­ها: این کارآزمایی بالینی تصادفی دو سوکور در سال ۱۴۰۳ بر روی ۸۴ کودک مراجعه‌کننده به اورژانس بیمارستان حکیم نیشابور انجام شد. کودکان به صورت تصادفی در سه گروه 28 نفره (طب فشاری، زنبورک ویبره و کنترل) قرار گرفتند. در گروه طب فشاری، نقطه هوگو به مدت 5 دقیقه قبل از تعبیه آنژیوکت فشرده شد؛ درگروه زنبورک ویبره، عروسک ویبره‌دار ۵ دقیقه قبل از تعبیه آنژیوکت روی دست کودک بسته شد؛ و در گروه کنترل، فقط اقدامات روتین انجام شد. داده‌ها با استفاده از پرسش‌نامه دموگرافیک و مقیاس واکنش‌های رفتاری ناشی از دردFLACC  جمع‌آوری و با نرم‌افزار SPSS نسخه ۲۶ تحلیل شدند.
یافته­ها: شدت درد در گروه‌ها تفاوت آماری معناداری داشت (P=0.000). زنبورک ویبره به‌طور معناداری کاهش درد بیشتری نسبت به طب فشاری(P=0.004)  و گروه کنترل (P=0.000) ایجاد کرد. نرخ موفقیت در تعبیه کاتتر وریدی در گروه زنبورک ویبره 100%، در طب فشاری 4/96% و در گروه کنترل 6/78% بود. زمان دسترسی نیز در گروه‌ها تفاوت معناداری نشان داد (P=0.000)؛ به‌طوری‌که زنبورک ویبره سریع‌ترین و طب فشاری سریع‌تر از گروه کنترل بود (P<0.01).
نتیجه‌گیری: طب فشاری و زنبورک ویبره هر دو به کاهش درد ناشی از تعبیه کاتتر وریدی کمک می‌کنند، اما زنبورک ویبره در کاهش درد، افزایش نرخ موفقیت و سرعت دسترسی به عروق، برتری دارد.

Comparison of the effect of acupressure and vibrating bee (Buzzy) on the success of the first access attempt and pain intensity of venous catheterization in 3 to 6 year-old children: A randomized clinical trial

Background: This study aimed to compare the effects of two specific techniques (acupressure and vibrating bee) on the first-attempt success rate of intravenous IV catheterization and the pain intensity experienced by children during the procedure.
Materials and Methods: This clinical trial was conducted in 2024 at the Hakim Hospital Emergency Department in Neyshabur, Iran, and involved 84 children. Following inclusion/exclusion criteria, participants were randomly assigned into three equal groups (n=28, balanced by gender): Acupressure (LI4 point pressed for 5 minutes before IV insertion), Vibrating bee (applied for 5 minutes before IV insertion), and a control group (standard venipuncture). Data, including demographics and FLACC pain scores, were collected and analyzed using SPSS 26.
Results: Statistical analysis revealed significant differences in pain intensity among the groups (P = 0.000). The vibrating toy bee method yielded lower pain scores compared to both the acupressure group (P = 0.004) and the control group (P = 0.000). Acupressure also proved more effective than the control group in reducing pain (P = 0.000). First-attempt success rates for catheter placement were 100% for the vibrating bee group, 96.4% for the acupressure group, and 78.6% for the control group, with no significant difference between acupressure and control (P = 0.051). The vibrating bee method demonstrated a significant overall advantage. Additionally, access time showed significant differences (P = 0.000), with the vibrating bee being the fastest, followed by acupressure, which was quicker than the control group (P < 0.0167).
Conclusion: Both the vibrating bee and acupressure methods reduced pain during IV insertion; however, the Vibration bee is more effective and faster, achieving a higher success rate (100% vs. 96.4% and 78.6%).
