چکیده

زمینه و هدف: افیوژن پلورال یکی از تظاهرات بالینی شایع است که می‌تواند نشانه‌ای از بیماری‌های سیستمیک یا موضعی مانند بدخیمی، عفونت، یا نارسایی قلبی باشد. تمایز بین افیوژن‌های ترانسوداتیو و اگزوداتیو برای تشخیص و درمان مناسب اهمیت بالایی دارد. هدف این مطالعه، بررسی ویژگی‌های آزمایشگاهی مایع پلور و ارتباط آن با نوع افیوژن بود.
مواد و روش­ها: این مطالعه توصیفی-مقطعی روی ۹۷ بیمار بستری با افیوژن پلورال در بیمارستان شهدای تجریش طی سال‌های ۱۳۹۸ تا ۱۴۰۲ انجام شد. اطلاعات بیماران از طریق چک‌لیست  مرور پرونده‌های پزشکی و آزمایشگاهی جمع‌آوری شد.
یافته­ها: نسبت پروتئین مایع به سرم (R²=0.87, p<0.05) و نتایج کشت با نوع مایع رابطه معناداری نشان دادند. مایعات اگزوداتیو بیشتر کشت مثبت داشتند. همچنین بین افزایش WBC و بروز سرطان در مایع پلور همبستگی مثبت معناداری یافت شد  (R²=0.62, p<0.05).
نتیجه‌گیری: بررسی‌های بیوشیمیایی مانند نسبت پروتئین، LDH و کشت می‌توانند به تشخیص نوع افیوژن کمک کنند. داده‌ها نقش کلیدی آنالیز ترکیبی بالینی و آزمایشگاهی در تشخیص دقیق افیوژن پلور را نشان دادند و بر ضرورت مطالعات آینده‌نگر برای اعتبارسنجی مارکرها تأکید دارند.

Introduction: Pleural effusion is a common clinical manifestation that may indicate serious systemic or localized diseases, including malignancy, infection, or heart failure. Accurate distinction between transudative and exudative effusions is crucial for effective diagnosis and treatment.
Materials and Methods: This descriptive cross-sectional study was conducted on 97 hospitalized patients with confirmed pleural effusion at Shohadaye Tajrish Hospital between 2019 and 2023. Data collection involved a review of medical records and laboratory reports, facilitated by a standardized checklist.
Results: A significant correlation was found between the pleural-to-serum protein ratio and the exudative nature of pleural fluid (R²=0.87, p<0.05). Additionally, positive microbial cultures were significantly more prevalent in exudative effusions. No significant difference was found in polymorphonuclear (PMN) cell counts between culture-positive and culture-negative samples. A positive association was noted between malignancy and elevated white blood cell (WBC) levels in pleural fluid (R²=0.62, p<0.05).
Conclusion: Biochemical parameters such as protein ratio, lactate dehydrogenase (LDH) levels, and culture results significantly contribute to the classification of pleural fluid. This study underscores the importance of an integrated diagnostic approach that combines laboratory markers, imaging, and clinical data. Further prospective, multicenter studies are recommended to validate these findings and support the routine implementation of such diagnostic strategies.
