چکیده
زمینه و هدف: بیماری لیشمانیوز به سه شکل پوستی، جلدی-مخاطی و احشایی است. هدف از این مطالعه، تعیین فراوانی و روند تغییرات زمانی لیشمانیوز پوستی در شهرهای تحت پوشش دانشگاه علوم پزشکی سبزوار طی سالهای ۱۳۹۳ تا ۱۴۰۰ بود.
مواد و روشها: بیماران با تشخیص نهایی لیشمانیوز پوستی که اطلاعات آنها در سیستم پورتال دانشگاه ثبتشده بود، ارزیابی شدند. برای تعیین نقاط داغ و نوع پراکنش بیماری از شاخص موران استفاده شد. همچنین، از نرمافزار SPSS 19 جهت تجزیهوتحلیل دادهها استفاده گردید. درنهایت برای مقایسه دادهها از نرمافزار کای مربع استفاده شد.
یافتهها: درمجموع 1483 بیمار با میانگین سنی 79/34 سال حضور داشتند. بیشترین شیوع (1/21 درصد) در سال 1393 و کمترین میزان (2/6 درصد) در سال 1397 مشاهده شد. شیوع بیماری در سبزوار (5/39 درصد) و جوین (3/22 درصد) بیشتر از سایر شهرها بود. آبان ماه با 401 مورد (1/27 درصد)، بیشترین میزان شیوع و فروردین با 19 مورد (28/1 درصد)، کمترین میزان را داشت. همچنین، فراوانی بیماری در جنس مذکر، 934 نفر (63 درصد) بیشتر از جنس مؤنث، 549 نفر (37 درصد) بود. بیشترین محل درگیری بدن نیز دست با 391 ابتلا (4/26 درصد) و پا با 259 ابتلا (5/17 درصد) بود.
نتیجهگیری: نتایج این مطالعه میتواند برای کمک به مسئولین بهداشتی در جهت انجام اقدامات پیشگیرانه و برنامهریزی برای کنترل بیماری در منطقه مفید باشد و از آن میتوان بهمنظور سیاستگذاری برای ارتقای بهداشت عمومی استفاده کرد.

Introduction: The incidence of cutaneous leishmaniasis in Iran is estimated at 15.4 cases per 100,000 people, with the highest prevalence reported in Ilam, Fars, and Razavi Khorasan provinces. This study aims to determine the frequency and temporal changes of cutaneous leishmaniasis in cities covered by Sabzevar University of Medical Sciences from 2014 to 2021.
Materials and Methods: Patients diagnosed with cutaneous leishmaniasis, whose information was recorded in the university portal system, were evaluated. Moran's Index was utilized to identify hotspots and disease-type distribution. Data analysis was performed using SPSS version 19, and chi-square tests were employed for data comparison.
Results: A total of 1,483 patients with an average age of 34.79 years were included in the study. The highest prevalence was observed in 2014 (316 cases), while the lowest was recorded in 2018 (92 cases). The prevalence in Sabzevar (586 cases) and Jovin (330 cases) was higher than in other cities. November had the highest prevalence rate (401 cases), whereas April had the lowest (19 cases). The incidence was higher in males (63%) was higher than in females (37%). The most affected parts of the body were hands (26.4%) and feet (17.5%).
Conclusion: The findings of this study can assist health authorities in implementing preventive measures and developing strategies to control cutaneous leishmaniasis in the region, ultimately contributing to public health improvement.
