چکیده

زمینه و هدف: آپلین نقش مهمی در تنظیم هموئوستاز انرژی دارد که نقش آن در دیابت و مقاومت به انسولین به‌درستی مشخص نیست بنابراین هدف این مطالعه، بررسی تأثیر تمرین ورزشی بر میزان آپلین سرمی و مقاومت به انسولین در بیماران مبتلا به دیابت نوع دو (T2DM) است.
مواد و روش­ها: جستجوی نظام‌مند برای مطالعات مرتبط در پایگاه‌های اطلاعاتی PubMed، Web of Science، SID، Magiran انجام شد. اندازه اثر و فاصله اطمینان ۹۵ درصد (CI) با استفاده از مدل اثر تصادفی محاسبه شد.
یافته­ها: تجزیه و تحلیل 14 مداخله با 386 بیمار T2DM نشان داد که تمرین ورزشی سبب تغییر معنادار آپلین سرمی ]8/0P= ،(731/0- تا 897/0) 083/0= [SMDنسبت به گروه کنترل در افراد بزرگسال نشد. با این وجود، تجزیه و تحلیل داده­­ها نشان داد که تمرین ورزشی سبب کاهش معنادار سطوح گلوکز ناشتا ]001/0P= ،(37/26- الی 69/11-)mg/dl  034/19-=[WMD، میزان انسولین ناشتا ]001/0P= ،(429/5- تا 804/1-)mU/mL  616/3-= [WMDو شاخص مقاومت به انسولین یا HOMA-IR ]001/0P= ،(044/2- تا 104/1-) 574/1-= [WMD نسبت به گروه کنترل در بیماران T2DM شد.
نتیجه‌گیری: نتایج فراتحلیل حاضر نشان داد که تمرین ورزشی تأثیری بر میزان آپلین سرمی ندارد ولی سبب کاهش گلوکز ناشتا، انسولین ناشتا و مقاومت به انسولین در بیماران T2DM می­شود؛ بنابراین، تمرینات ورزشی یک روش مناسب و کارآمد برای بهبود مقاومت به انسولین در بیماران T2DM است.

The effect of exercise training on serum apelin levels and insulin resistance in patients with type 2 diabetes: a systematic review and meta-analysis

Introduction: Apelin is crucial for regulating energy homeostasis, yet its role in diabetes and insulin resistance remains inadequately understood. This study aimed to investigate the effects of exercise training (Exe) on serum apelin levels and insulin resistance in patients with type 2 diabetes mellitus (T2DM).
Materials and Methods: A systematic search for relevant studies was conducted using the PubMed, Web of Science, SID, and Magiran databases. Effect sizes and 95% confidence interval (CI) were calculated using a random effects model.
Results: Analysis of 14 interventions involving 386 adults with T2DM indicated that exercise training did not result in a significant change in serum apelin levels [SMD=0.083 (95% CI: 0.731 to 0.897), P=0.8] compared to the control group. However, exercise training was associated with significant reductions in fasting glucose levels [WMD=-19.034 mg/dl (95% CI: -11.69 to -26.37), P=0.001], fasting insulin levels [WMD=-3.616 μU/ml (95% CI: -1.804 to -5.429), P=0.001], and insulin resistance as measured by HOMA-IR [WMD=-1.574 μU/ml (95% CI: -1.104 to -2.044), P=0.001], relative to the control group.
Conclusion: The results of this meta-analysis indicate that exercise training does not significantly affect serum apelin levels. However, it contributes to reductions in fasting glucose, fasting insulin, and insulin resistance in patients with T2DM. Thus, exercise is an effective strategy for improving insulin resistance in individuals with T2DM.
