چکیده

زمینه و هدف  کم‌خونی فقر آهن به علت شیوع بالا و تأثیرات نامطلوب به عنوان یکی از مشکلات اصلی بهداشت عمومی در بارداری مطرح شده است؛ لذا مطالعه حاضر با هدف بررسی رابطه بین کم‌خونی فقر آهن مادر و تأثیر آن بر وزن هنگام تولد نوزاد در جمعیت تحت پوشش دانشگاه علوم پزشکی مشهد در سال 1397 انجام شد.
مواد و روش‌ها در پژوهش توصیفی- تحلیلی حاضر، اطلاعات مربوط به تولد نوزادان از سامانه پرونده الکترونیک سلامت سینا استخراج شد. در این مطالعه، مادران باردار به دو گروه مبتلا به کم‌خونی و سالم، چهار گروه کم‌خونی متوسط تا شدید، خفیف، طبیعی و هموگلوبین بالا و نه گروه با فواصل 10 گرم بر لیتر از کم‌خونی شدید تا هموگلوبین بالا تقسیم‌بندی شدند. متغیرهای این پژوهش شامل سن، تحصیلات، تعداد فرزندان، وزن، قد و دور سرنوزادان بود.تجزیه‌وتحلیل داده‌ها با استفاده از نرم‌افزار آماریSTATA  نسخه 14 و با استفاده از روش‌های آمار توصیفی، آزمون‌ تست تی و رگرسیون لجستیک انجام شد. سطح معنی‌داری در این مطالعه 0/05درنظر گرفته شد.
یافته‌ها نتایج این مطالعه نشان داد غلظت کم هموگلوبین با نسبت احتمال بالای تولد نوزاد کم‌وزن (0/271=P ، 1/19=OR)، دورسر (0/611= P، 1/07= OR) و قد (0/587=P ، 1/10=OR) غیرطبیعی نسبت با هموگلوبین طبیعی مرتبط است. خطر این عوامل در 9 گروه هموگلوبین مادران باردار به شدت کم‌خونی بستگی دارد. در این مطالعه، غلظت هموگلوبین بالا با خطر تولد نوزاد کم‌وزن، دورسر و قد غیرطبیعی، مرتبط است.
نتیجه‌گیری نتایج این مطالعه نشان داد که غلظت‌های کم و زیاد هموگلوبین مادر در طول بارداری می‌تواند تأثیر به‌سزایی در قد و وزن نوزاد داشته باشد.

چکیده [English]

Introduction: Iron deficiency anemia Has been raised due to high prevalence and adverse effects as one of the main problems of public health in pregnancy. The aim of this study was to investigate the relationship between maternal iron deficiency anemia and its effect on birth weight in a population covered by Mashhad University of Medical Sciences in 2018.
Materials and Methods: In this descriptive-analytic research, information on the birth of newborns was extracted from the Sina Electronic Health Record System. In this study, pregnant mothers were divided into two groups: anemia and non-anemia, 4 groups: moderate to severe, mild, normal and high hemoglobin, and 9 groups with 10 g/L interval from severe anemia to high hemoglobin. Data analysis was performed using statistical software STATA 14 and descriptive statistics, t test and logistic regression. The significance level in this study was considered 0.05.
Results: Result of this study showed Low concentration of Hemoglobin was associated with higher odds of Low Birth Weight (OR:1.19 P:0.271) abnormal head circumference (OR:1.07 P:0.611) and abnormal hight (OR:1.10 P:0.587) When compared with normal hemoglobin. The risk of this factors across 9 pregnancy hemoglobin groups depended on severity of anemia. In this study high hemoglobin concentration was associated with an elevated risk of Low Birth Weight, abnormal head circumference and abnormal hight of neonate.
Conclusion: The results of this study showed that high and Low maternal hemoglobin concentrations during pregnancy can have an adverse effect on neonatal weight and height.
