چکیده
اهداف با وقوع مرگ مادران باردار، بنیان خانواده و سلامت فرزندان در معرض خطر قرار می گیرد. شناخت عوامل مؤثر بر این شاخص مهم، ما را در اتخاذ راهکارهایی برای پیشگیری از بروز مرگهای مشابه و بهبود آن، یاری میکند. هدف این پژوهش، تعیین فراوانی و عوامل مؤثر بر مرگومیر مادران باردار در شهر سبزوار طی دوره دهساله (1382-1392) بود.
مواد و روش ها مطالعه حاضر یک مطالعه توصیفیتحلیلی از نوع مقطعی بود و جامعه پژوهش آن شامل همه مادران بارداری بود که از فروردین سال1382 تا پایان اسفندماه سال 1392 در شهر سبزوار فوت کرده بودند. گردآوری اطلاعات از طریق مراجعه به کمیته مرگومیر مادری شهر سبزوار و همچنین مطالعه پرونده بیمارستانی آنها و تکمیل فرم اطلاعاتی مربوط به مرگو میر مادران باردار انجام شد. سپس دادهها وارد نرمافزار SPSS نسخه 5/11 شد و توسط روشهای آمار توصیفی و کای دو تجزیهوتحلیل شد. سطح معناداری نیز 0.05 در نظر گرفته شد.
یافته ها تعداد مرگومیر در زنان مطالعهشده 20 مورد بود. بیشترین تعداد متوفیان مربوط به ساکنان شهر (70 درصد) بودند که از نظر آماری نیز آزمون کای دو نشان داد میان علت مرگ و محل سکونت رابطه معناداری وجود دارد. (P=0/04).80 درصد افراد فاقد فرم ارجاع از مراکز بهداشتی بودند، 60 درصد افراد نیز بهدنبال زایمان به روش سزارین فوت شده بودند. شایعترین علت مرگومیر در افراد مطالعهشده خونریزی (30 درصد) و سابقه بیماری زمینه ای (30 درصد ) بود.
نتیجه گیری عواملی نظیر رعایت تنظیم خانواده و عدم بارداری در سنین خیلی زیاد، آموزش به مادران درباره خطرات سزارین و مزیت زایمان طبیعی و افزایش سواد بهداشتی آنان، در کاهش مرگومیر مادران باردار نقش بسزایی دارد.

چکیده [English]
Objectives The death of pregnant mothers endangers the health of children and the foundation of the family. Learning about the factors affecting this important issue helps to adopt strategies to prevent similar cases. The current study aimed at determining the prevalence and risk factors of maternal mortality in Sabzevar, Iran, over a period of 10 years (2003-2013).
Methods The study population of the current cross sectional, descriptive survey included all pregnant mothers died in Sabzevar from April 2003 to March 2013. Data were collected by referring to the Sabzevar Committee of Maternal Mortality. The medical records were investigated and datasheets were completed for the subjects; then, the data were transferred to SPSS software version 5.11; data were analyzed using descriptive statistics and Chi-square test. The significance level was 0.05.
Results The frequency of death in the current study was 20 . The Chi-square test showed statistically significant relationship between the cause of death and place of residence in the studied cases (P-value = 0.04). A total of 80% of the cases had no referral form or file in the health centers; also, 60% of the pregnant cases looking for the caesarean section had died. The most common causes of death in the cases were bleeding (30%) and the underlying diseases (30%).
Conclusion Factors such as family planning, preventing higher ages pregnancy, educating mothers on the risks and benefits of vaginal delivery and caesarean, and increasing health literacy play important role in reducing the maternal mortality.
