چکیده
اهداف آپاندیسیت یکی از شایعترین و مهمترین اورژانسهای جراحی است. بیشتر این بیماران تحت عمل جراحی آپاندکتومی قرار میگیرند. تشخیص آپاندیسیت امری حساس است و ارزش تشخیصی آزمایشها همواره مورد بحث و تبادلنظر بوده است. هدف این مطالعه تعیین ارزش تشخیصی افزایش پروتئین فعال سی در تشخیص آپاندیسیت حاد است.
مواد و روش ها مطالعه حاضر از نوع توصیفیمقطعی بود. بدین منظور 105 نفر از مراجعهکنندگان به بخش اورژانس بیمارستان شهیدمطهری مرودشت در سال 1391 ارزیابی شدند. قبل از دریافت آنتیبیوتیک پیش از عمل، برای اندازهگیری سطح سرمی پروتئین فعال سی از آنها نمونه خون گرفته شد. پس از جراحی ارتباط میان سطح سرمی پروتئین فعال سی با جواب پاتولوژی بافت آپاندیس و نظر جراح درباره نوع بافت بررسی شد. اطلاعات بهدستآمده با نرمافزار SPSS نسخه 18 و آزمون تحلیل واریانس و آزمون توکی تجزیهوتحلیل شد.
یافته ها 81 نفر از افراد مطالعهشده آپاندیسیت حاد و 24 نفر آپاندیسیت عادی داشتند. از میان افراد مبتلا به آپاندیسیت حاد، در 65 نفر سطح سرمی پروتئین فعال سی افزایش یافته و در 16 نفر سطح سرمی پروتئین فعال سی عادی بود. در این مطالعه حساسیت و ویژگی پروتئین فعال سی به ترتیب 80 درصد و 63 درصد بود.
نتیجه گیری نتایج این مطالعه نشان داد سنجش سطح سرمی پروتئین فعال سی میتواند در تشخیص آپاندیسیت حاد سودمند باشد.

Relationship Between Serum Levels of C-Reactive Protein and Symptoms of Acute Appendicitis in Patients With Acute Appendicitis

Objectives Appendicitis is the most common and important emergency condition, in which most of the patients undergo appendectomy. The diagnosis of appendicitis is critical and the values of diagnostic laboratory tests are always discussed. The current study aimed at determining the diagnostic value of C-Reactive Protein (CRP) increase in the diagnosis of acute appendicitis.
Methods The current cross sectional study examined 105 individuals referred to the emergency department of Shahid Motahari Hospital, Marvdasht, Iran, in 1012. Before the administration of any preoperational antibiotics, blood samples were obtained from the subjects to measure the serum CRP level. After surgery, the relationship among serum levels of CRP and appendix tissue biopsy, and the surgeon`s comments about the type of tissue were investigated. All data were analyzed by SPSS version 18, employing ANOVA and Tukey test.
Results A total of 81 patients had acute appendicitis, out of which 24 had normal appendix. Among patients with acute appendicitis, 65 subjects showed increased serum CRP level and 16 subjects had normal serum CRP level. The sensitivity and specificity of CRP level in the study were 80% and 63%, respectively.
Conclusion The results showed that evaluation of serum CRP levels can be useful and beneficial to diagnose acute appendicitis.
