چکیده

زمینه و هدف: با توجه به مطالعات اندک در زمینه وسواس های فکری و وجود درمان های غیردارویی، می توان درمان فراشناختی را به عنوان گزینه احتمالی مؤثر با اثرات بلندمدت تر معرفی کرد. هدف پژوهش حاضر مقایسه اثربخشی درمان فراشناختی و درمان دارویی بر روی مبتلایان به وسواس های فکری در اختلال وسواسی- اجباری می باشد.
مواد و روش ها: این پژوهش از نوع شبه تجربی با گروه کنترل و مورد می باشد . جهت اجرای طرح، افراد به طور مساوی در دو گروه 8 نفره و تحت دو مدل گروه درمانی فراشناختی (14جلسه) و درمان دارویی به (مدت 1 ماه) طبقه بندی شدند. در ابتدا و انتهای درمان، از اعضای هر دو گروه با کمک پرسشنامه های فراشناخت (MCQ-30)، مقیاس وسواسی- اجباری ییل براون (Y-BOCS)، کنترل فکر (TCQ)، مقیاس اضطراب، افسردگی، استرس (DASS-21)، پرسشنامه افسردگی بک (BDI-II) و مقیاس صفت اضطراب از پرسشنامه اضطراب صفت و حالت اشپیلبرگر (STAY-T) و مصاحبه برای ارزیابی عملکرد کلی، پیش آزمون و پس آزمون به عمل آمد. داده ها به وسیله نرم افزار آماری SPSS 18 و آزمون های آماری تی و کوواریانس مورد بررسی قرار گرفت.
یافته ها: در کاهش شدت وسواس های فکری (006/0=P) با میانگین های پیش آزمون 07/2±13/50 و پس آزمون 81/2±25/10، استرس (003/0=P) با میانگین های پیش آزمون 89/4±00/30 و پس آزمون 52/3±87/24 و متغیر باورهای فراشناختی منفی (001/0=P) با میانگین های پیش آزمون (72/2±00/15) و پس آزمون 25/2±75/11 استفاده از درمان فراشناختی ارجحیت دارد؛ همچنین ارتباط معناداری نیز بین افزایش عملکرد کلی و دارو درمانی (042/0=P) با میانگین های پیش آزمون 83/4±25/60 و پس آزمون 28/5±75/61 دیده می شود. مشاهدات بیانگر آن بود که بین متغیرهای افسردگی (09/0=P)، نگرانی (07/0=P) و اضطراب (2/0=P) تفاوت معناداری بین درمان های مذکور وجود ندارد.
نتیجه گیری: یافته ها نشان می دهد که کاهش شدت وسواس های فکری، استرس و باورهای فراشناختی منفی با استفاده از روش درمانی فراشناختی، نتایج بهتری نسبت به درمان دارویی دارد. اما نتایج در خصوص افزایش عملکرد کلی به نفع درمان دارویی می باشد. ارتباط معناداری نیز بین دو نوع درمان استفاده شده جهت متغیرهای افسردگی، نگرانی و اضطراب وجود ندارد.

Comparing the Metacognitive and Pharmaceutical Therapies of the Obsessive Compulsive Disorder

چکیده [English]

Background and Purpose: Due to the small number of studies on obsessive compulsive disorder and the existence of non-pharmaceutical treatments، it is possible to introduce the Metacognitive therapy as an alternative with long-term effects. The purpose of present study was to compare the effectiveness of metacognitive therapy versus pharmaceutical therapy on the patients with obsessions in Obsessive-Compulsive Disorder.

Methods and materials: This research is a quasi-experimental design with experimental and control groups. The participants were equally assigned into two groups with 8 members، metacognitive therapy (14 sessions) and pharmaceutical therapy (one month). Pretest and posttest were given to both groups: MCQ30، YBOCS، DASS21، BDI&II، STAY-T. The obtained data were analyzed in SPSS 18 using t-test، chi-square and covariate analysis.

Results: The results showed a preference for metacognitive therapy in decreasing obsession intensity (P=0.006) with pretest mean of 50.13±2.07 and posttest mean of 10.25±2.81، negative variables of metacognition (P=0.001) with pretest mean of 15.00±2.72 and posttest mean of 11.75± 2.25. A significant relationship was observed between the general functioning and the pharmaceutical therapy (P=0.042) with pretest mean of 60.25±4.83 and posttest mean of61.75±5.28. However، no significant differences were observed between the two types of therapy as far as depression (0=0.09)، worry (p=0.07) and anxiety (p=0.2) are concerned.

Conclusion: The results indicated better results with metacognitive therapy in reducing the obsessions، stress and negative metacognitive beliefs. However، enhancing the overall functioning of the patients can be achieved with pharmaceutical therapy. There were no signification correlation between the two types of therapy for treating depression، worry and anxiety.
