چکیده

زمینه و هدف: فرایندهای روانی و هیجانی زیادی می­توانند در آسیب­پذیری نسبت به سندرم روده تحریک‌پذیر دخیل باشند. هدف پژوهش حاضر، تبیین رابطه علّی بین طرح‌واره­های هیجانی و شدت علائم در بیماران مبتلا به سندرم روده تحریک‌پذیر براساس نقش میانجیگر خشم بود.
مواد و روش‌ها: روش پژوهش، توصیفی از نوع همبستگی و جامعه آماری پژوهش حاضر، تمام بیماران مبتلا به سندرم روده تحریک­پذیر مراجعه‌کننده به کلینیک گوارش بیمارستان امام رضا شهر مشهد در سال1399 بود که از میان آنها323 نفر به روش نمونه‌گیری در دسترس انتخاب شدند. ابزار پژوهش شامل مقیاس طرح‌واره هیجانی لیهی (2002)، پرسش‌نامه شدت علائم روده تحریک­پذیر فرانسیس و همکاران (1997) و مقیاس خشم نواکو (1994) بود. داده­ها به روش مدل‌یابی معادلات ساختاری تحلیل شدند.
یافته‌ها: در پژوهش حاضر، ضریب مسیر کل بین طرح‌واره‌‌های هیجانی غیرانطباقی و شدت علائم سندرم روده تحریک‌پذیر (0/01>p، 0/303=β) مثبت و در سطح 0/01 معنادار بود. ضریب مسیر کل بین طرح‌واره‌‌های هیجانی انطباقی و شدت علائم سندرم روده تحریک‌پذیر (0/01>p، 0/264-=β) منفی و در سطح0/01 معنادار بود. ضریب مسیر مستقیم بین خشم و شدت علائم سندرم روده تحریک‌پذیر (0/01>p، 0/505=β) مثبت و در سطح0/01 معنادار بود. ضریب مسیر غیرمستقیم بین طرح‌واره­های هیجانی غیرانطباقی و شدت علائم سندرم روده تحریک‌پذیر (0/01>p، 0/128=β) مثبت و در سطح0/01 معنادار و ضریب مسیر غیرمستقیم بین طرح‌واره­های هیجانی انطباقی و شدت علائم سندرم روده تحریک‌پذیر (0/01>p، 0/139-=β) منفی و در سطح0/01 معنادار بود.
نتیجه‌گیری: نتایج پژوهش حاضر نشان داد خشم رابطه بین طرح‌واره­های هیجانی انطباقی و غیرانطباقی با شدت علائم سندرم روده تحریک‌پذیر را میانجیگری می‌کند

چکیده [English]

Introduction: Many psychological and emotional processes can be involved in the vulnerability to irritable bowel syndrome (IBS). This study aimed to explain the causal relationship between emotional schemas and the severity of symptoms in patients with irritable bowel syndrome based on the mediatory role of anger. Materials and Methods: The descriptive-correlational method was used in this study for data collection. The statistical population consisted of all patients with irritable bowel syndrome referred to Imam Reza Hospital's Gastrointestinal Clinic in Mashhad in 2020, from whom 323 individuals were selected by convenience sampling. Research tools included Irritable Bowel Syndrome Severity Scoring System (IBS-SSS) by Francis et al. (1997), Novaco Anger Scale (NAS) (1994), and Leahy Emotional Schema Scale (LESS) (2002). The data were analyzed by structural equation modeling.
Results: The indirect pathway coefficient between non-adaptive emotional schemas and the severity of irritable bowel syndrome symptoms (p<0.01, β=0.128) was positive and significant at the level of 0.01. Also, the indirect path coefficient between adaptive emotional schemas and severity of irritable bowel syndrome symptoms (p<0.01, β=-0.139) was negative and significant at the level of 0.01.
Conclusion: The present study results show that anger mediates the relationship between adaptive and non-adaptive emotional schemas with the severity of irritable bowel syndrome symptoms.
