چکیده

مقدمه :امروزه کلبسیلا پنومونیه به عنوان یک پاتوژن فرصت طلب در عفونت های بیمارستانی مطرح می باشد.
طیف وسیعی از بتالاکتامازها در سراسر جهان در حال افزایش می باشد. آنتی بیوتیک های بتالاکتام جزء مرسوم ترین عوامل ضد میکروبی در کنترل و درمان عفونت های باکتریایی محسوب می شوند. هدف از این مطالعه ، تعیین نقش ژن های CTX-M-2،CTX-M-8 و CMY-2در سویه های کلبسیلا پنومونیه مولد بتالاکتاماز های وسیع الطیف تولید شده در بیمارستان های تهران می باشد.
مواد و روش :در این مطالعه توصیفی، تعداد 60 نمونه بالینی از بیمارستان های مختلف تهران جمع آوری و با آزمون های بیوشیمیایی شناسایی گردید. تست حساسیت آنتی بیوتیکی به روش دیسک دیفیوژن با توجه به استاندارد 2016CLSI انجام شد به منظور شناسایی بتالاکتاماز از تست سینرژیسیم دوبل استفاده شد و جهت شناسایی ژن های بتالاکتاماز از آزمون Multiplex-PCR استفاده گردید.
یافته ها :در این تحقیق، بیشترین مقاومت نسبت به آنتی بیوتیک آمپی سیلین و بیشترین حساسیت نسبت به آنتی بیوتیک آمیکاسین تعیین گردید. از60 نمونه، مورد بررسی، 33 ایزوله حاوی ژنCMY-2 ، 8 ایزوله حاوی ژنCTX-M-2 و 25 ایزوله حاوی ژن CTX-M-8 می باشد. نتایج کار آماری بر اساس نرم افزار SPSS نشان داد ارتباط بین ژن های مورد مطالعه و مقاومت آنتی بیوتیکی معنا دار می باشد (05/0p value

چکیده [English]

Backgeround: Klebsiella pneumoniae is an opportunistic pathogen and is one of the most important bacteria involved in hospital infections.The broad spectrum of beta-lactatmase is increasing worldwide. Antibiotics of β-lactams are among the most common antimicrobial agents in controlling and treating bacterial infections. The aim of this study was to determine the CTX-M-2, CTX-M-8 and CMY-2 genes in Klebsiella pneumoniae strains producing broad-spectrum ß-lactamase isolated from Tehran hospitals.
Methods: In present study, 60 samples were collected from different hospitals in Tehran and were identified by biochemical tests. Antibiotic susceptibility test was performed using CLSI absolute diffusion method. Also, To detect beta-lactamases, double-synergistic test was used. the multiplex-PCR was utilized to detect beta-lactamase genes.
Results: In this study, the highest resistance to ampicillin antibiotics was determined .Also, the highest sensitivity to antibiotic amikacin was .Among 60 samples, 33 samples contained the CMY-2 gene, 8 samples contained the CTX-M-2 gene and 25 samples contained the CTX-M-8 gene.The results of the investigationon the basis of SPSS software showed that the relationship between the studied genes and antibiotic resistance is significant.(p value
