چکیده

اهداف: تداخل درد یکی از ویژگی‌های مهم درد مزمن است که تعیین کننده نوع و مسیر درمان است. با این‌حال دربارة‌ تاثیر مداخلات بر تداخل درد اطلاعات محدودی موجود است. ذا هدف از پژوهش حاضر تعیین تاثیر شناخت درمانی مبتنی بر حضور ذهن (MBCT) بر تداخل درد در بیماران مبتلا به سردرد اولیه بود.
مواد و روش‌ها: در پژوهش آزمایشی حاضر در سال 1396 الی 97 در بیمارستان امام حسین (ع) در استان تهران انجام شد. از بین 94 بیمار مبتلا به سردرد مزمن اولیه 85 نفر با استفاده از روش نمونه‌گیری هدفمند انتخاب و به دو گروه درمان MBCT (43 نفر) و گروه کنترل (42 نفر) به صورت تصادفی جای‌گماری شده و در دو مرحله ارزیابی شدند. گروه آزمایش در هشت جلسة 120 دقیقه‌ای و در هفته‌های متوالی در جلسات درمانی MBCT شرکت کردند و گروه کنترل از گفت‌وگوی معمول با درمانگر برخوردار شدند. از مقیاس پیوستة‌ بصری(NRS)، و پرسشنامه مختصر درد (BPI) استفاده شد. داده‌ها از طریق آزمون تحلیل کوواریانس چند متغیره، تجزیه و تحلیل شدند. نتایج در دو سطح «تحلیل به قصد درمان» و نمونه افراد تکمیل کننده پژوهش گزارش شدند.
یافته‌ها: نتایج «تحلیل به قصد درمان» نشان داد که در مقایسه با گروه کنترل، تفاوت نمره پیش آزمون-پس آزمون در حیطه تداخل عاطفی درد( 001< P و 81/39=F) معنادار است. نتایج افراد تکمیل کننده پژوهش نیز مشابه و حاکی از کاهش معنادار در دو حیطه شدت درد (001< P و 75/10=F)، تداخل عاطفی درد (001< P و 69/81=F) می‌باشد.
نتیجه‌گیری: درمان MBCT یک روش بالقوه موثر برای کاهش شدت و تداخل درد عاطفی در افراد مبتلا به سردردهای مزمن است.

Background:
Pain interference is one of the important characteristics of chronic pain that determines the type and route of treatment. However, limited information is available about interventions efficacy on pain interference. Therefore, the aim of this study was to evaluate the effect of Mindfulness-based Cognitive Therapy (MBCT) on activity and emotional pain interference in patients with primary headache.
Methods: A clinical trial was conducted in 2017 and 2018 Imam Hossein hospital in the city of Tehran, Iran. Then, using the sampling judgment among 94 in patients with chronic headache, 85 were selected and randomly assigned into two groups: MBCT (43 patients) and control group (42) and were evaluated in two stages. The Numeric pain rating scale (NRS) Brief Pain Inventory (BPI), were measured. All data were analyzed by multivariate analysis of covariance. The results were reported using intent-to-treat (ITT) analyses and completer samples.
Findings: In ITT analyses statistically significant were found for emotional pain interference (P
