چکیده

زمینه و هدف: حفظ فشار مناسب کاف لوله تراشه در طول مدت بستری بیمار در بخش مراقبت‎های ویژه حائز اهمیت است. از عوامل تاثیر گذار بر فشار کاف لوله تراشه بیماران اقدامات مختلف پرستاری می‎باشد. این مطالعه با هدف تعیین تاثیر تغییر وضعیت بدن بر فشار کاف لوله تراشه بیماران بستری در بخش مراقبت های ویژه انجام شد.
مواد و روش‎ها: این مطالعه یک کارآزمایی بالینی است. جامعه‎ی پژوهش در این مطالعه بیماران بستری در بخش مراقبت‎های ویژه بود که دارای لوله تراشه دهانی و متصل به دستگاه تهویه مکانیکی بودند. در پژوهش حاضر 29 بیمار به روش مبتنی بر هدف انتخاب شدند. برای جمع آوری اطلاعات از چک لیست اطلاعات دموگرافیک و چک لیست ثبت اطلاعات مربوط به فشار کاف لوله تراشه بیماران و معیار بررسی آژیتاسیون-سداسیون ریچموند استفاده گردید. تغییرات فشار کاف لوله تراشه، پس از تغییروضعیت‎ بدن به سمت چپ و راست در زاویه‎های تخت 30 درجه و 45 درجه بلافاصله، اندازه‎گیری و ثبت می‎گردید. اطلاعات با استفاده از نرم افزار SPSS 19 و SAS و آزمون‎های آماری توصیفی، آزمون پیرسون ،آزمون تی مستقل ،آنالیز واریانس و با سطح معناداری 0.05 تجزیه و تحلیل شد.
یافته‎ها: نتایج این مطالعه نشان داد که بین عوامل دموگرافیک (سن، جنس، علت بستری) با تغییرات فشار کاف لوله تراشه رابطه معنادار آماری مشاهده نشد. اگرچه یافته‎های این پژوهش حاکی از وجود رابطه‎ی معنادار آماری بین فشار کاف لوله تراشه در زاویه‎ی 30 درجه با فشار کاف در تغییر وضعیت‎های بدن به چپ (p=0.004) و راست (p=0.00) و همچنین تغییر زاویه‎ی تخت به 45 درجه و راست (p=0.012) در این زاویه، بود.
نتیجه‎گیری: فشار کاف لوله تراشه با عوض کردن وضعیت بیمار، تغییر میکرد. اندازه‎گیری فشار کاف و تنظیم فشار آن در محدوده درمانی توصیه می‎شود.

چکیده [English]

Background: It is important to maintain proper endotracheal tube cuff pressure during hospitalization in intensive care units (ICUs). Among variables affecting endotracheal tube cuff pressure (p.cuff) in patients is various nursing practices. So, this study assessed the effect of body position changes on p.cuff in patients hospitalized at the ICU.
Materials and methods: In this clinical trial, mechanically ventilated patients with oral endotracheal tube hospitalized at the ICU were enrolled. The patients were selected using the purposive sampling method. A demographic data checklist and Richmond Agitation Sedation Scale were used to collect the data. p.cuff changes, after changing body position to the left and right at bed angles of 30° and 45°, were immediately measured and recorded. The data were analyzed using the SAS and SPSS 19 software packages along with descriptive statistical tests, chi-square test, independent t-test and ANOVA at the significance level of 0.05.
Results: The results of this study showed no significant relationship between demographic variables and endotracheal tube cuff pressure changes. The results indicated a significant relationship between endotracheal tube cuff pressure at 30-degree bed angle and cuff pressure in the body position changes to the left (p = 0.004) and right (p = 0.00), as well as between 45‎° head of bed and cuff pressure in the body position changes to right (p = 0.012).
Conclusion: The endotracheal tube cuff pressure was observed to change by changing the patient's position, change. Therefore, it is recommended to measure p.cuff and regulate it within the therapeutic range.
