چکیده

زمینه و هدف استرپتوکوکوس پیوژنز شایع‌ترین عامل باکتریایی فارنژیت است. بیماران مبتلا به گلودرد استرپتوکوکی باید به سرعت با آنتی‌بیوتیک‌های مناسب درمان شوند. در صورت عدم درمان مناسب عوارضی از قبیل تب روماتیسمی و گلومرولونفریت حاد ایجاد خواهد شد. همچنین مصرف نابجای آنتی‌بیوتیک نیز باعث ایجاد مقاومت آنتی‌بیوتیکی می‌شود. روش استاندارد جهت تشخیص استرپتوکوکوس پیوژنز، کشت سواب گلو است که وقتگیر می‌باشد. بنابراین ابداع یک کیت جدید، ارزان، سریع و مناسب جهت تشخیص استرپتوکوکوس پیوژنز از نمونه‌های گلو حائز اهمیت ویژه می‌باشد.
مواد و روش‌ها در این پژوهش یک کیت جدید به منظور تشخیص سریع استرپتوکوکوس پیوژنز طراحی گردید. 24 جدایه استرپتوکوکوس پیوژنز با تست لاتکس آگلوتیناسیون و کیت جدید بررسی شدند. با در نظر گرفتن نتایج روش لاتکس آگلوتیناسیون، حساسیت و اختصاصیت کیت جدید محاسبه شد. همچنین جهت بررسی اثر کیت جدید بر فلور نرمال گلو، 30 نمونه از دانشجویان دانشگاه الزهرا (س) به دست آمد.
یافته‌ها 24 جدایه با روش لاتکس آگلوتیناسیون به عنوان استرپتوکوکوس پیوژنز تشخیص داده شد. با استفاده از کیت جدید نیز همان نتایج به دست آمد (κ = 1). حساسیت و اختصاصیت کیت جدید به ترتیب 100% و 100% بودند. رشد فلور نرمال گلو در خانه دوم پلیت سه خانه مهار شد.
نتیجه‌گیری کیت جدید زمان و هزینه لازم جهت تشخیص استرپتوکوکوس پیوژنز از نمونه‌های گلو را کاهش می‌دهد. همچنین می‌تواند امکان درمان سریع با آنتی‌بیوتیک‌های مناسب را فراهم کند و از مقاومت آنتی‌بیوتیکی و عوارض حاصل از مصرف نابجای آنتی‌بیوتیک و همچنین از ایجاد تب روماتیسمی و گلومرولونفریت حاد جلوگیری ‌کند.

چکیده [English]

Backgrounds Streptococcus pyogenes is the most common cause of bacterial pharyngitis. Patients with streptococcal pharyngitis should be rapidly treated with appropriate antibiotics. Rheumatic fever and acute glomerulonephritis can develop as complications of inadequately treated streptococcal pharyngitis. Furthermore, antibiotic misuse promotes antibiotic resistance. Throat swab culture is standard diagnostic method for detection of S.pyogenes, which is time-consuming. Thus, developing a new, rapid, appropriate and inexpensive kit is very important for detection of S.pyogenes from pharyngeal specimens.
Methods and Materials In this study a new kit was designed for rapid detection of S.pyogenes. Twenty four S.pyogenes isolates were examined with both latex agglutination test and new kit. Sensitivity and specificity of new kit were calculated considering latex agglutination test results. Thirty sample swabs were also obtained from students of Alzahra University for examination of effect the new kit on normal throat flora.
Results Twenty four S.pyogenes isolates were identified as S.pyogenes using latex agglutination test and same results were obtained using new kit (κ = 1). The sensitivity and specificity of our new kit were 100% and 100%, respectively. Growth of the normal flora was inhibited in the second section of three section plate.
Conclusion New kit can decrease the required time and expense for detection S.pyogenes from pharyngeal specimens. It can also provide possibility rapid treatment with appropriate antibiotics and prevents rheumatic fever, acute glomerulonephritis, unwanted side effects of antibiotic misuse and antibiotic resistance.
