چکیده

مطالعات نشان داده اند که رکانالیزاسیون سریع عروق مغزی در بیماران دچار سکته مغزی موجب بهبودی و عملکرد بهتر این بیماران می گردد. رکانالیزاسیون عروق مغزی یا به کمک تزریق وریدی داروهای ترومبولیتیک (rtPA) یا به کمک روشهای اندوواسکولار انجام می شود. تزریق وریدی rtPA در 3 ساعت اول شروع علایم موجب بهبودی پیشاگهی بیماران سکته مغزی ایسکمیک می گردد ولی پاسخ آن بر روی عروق بزرگ ناچیز می باشد. مطالعات اخیر نشان دادند که روشهای آندوواسکولار و حذف مکانیکال لخته در عروق بزرگ مغز در بیماران سکته های مغزی در 6 ساعت اول شروع علائم موجب بهبودی این بیماران و عملکرد انها در دراز مدت می گردد. در حال حاضر ترومبکتومی مکانیکال قدم اول درمان حاد سکته های مغزی در بسیاری از کشورها و گایدلاینها می باشد. روشهای مختلفی برای انجام آن بوجود امده است که هر کدام فواید و مضرات خاص خود را دارند. بنابراین شناخت روشهای مختلف ترومبکتومی و انتخاب مناسب انها قدم مهمی در درمان حاد سکته های مغزی می توانند باشد.

Background & Objectives: Several studies confirmed that early recanalization of intracranial arteries during ischemic strokes result in better recovery of these patients after three months of treatment.
Materials & Methods: Recanalization of cerebral arteries is performed using intravenous tPA or mechanical thrombectomy.
Results: Intravenous tPA injection within 3 hours of ischemic stroke is associated with better outcome in three months follow-up. Intravenous tPA has less effect on large cerebral arteries, like internal carotid arteries. Recent studies showed that mechanical thrombectomy of large cerebral arteries within 6 hours of stroke onset is associated with better outcome three months after operation. Since 2015, mechanical thrombectomy has been entered in first-line of acute phase of ischemic stroke in several guidelines and countries. Mechanical thrombectomy included several different endovascular techniques, as thrombosuction and stent retrieval for clot removal.
Conclusion: Understanding several these different techniques, thier benefits, and their safety could help better selection of suitable method for endovascular treatment of stroke patients.
