زمینه و هدف: توجه به درد زایمان و تسکین آن از اجزا مهم مراقبت از مادر طی زایمان است. به نظر می رسد تغییر وضعیت مادر بر شدت درد زایمان موثر باشد. وضعیت هایی که مادر جهت راحتی بیشتر انتخاب می کند، غالبا سبب بهبود نزول جنین در کانال زایمان می شوند. اغلب هنگامی که جنین به خوبی با لگن مادر تطابق دارد، مادر درد کمتری را ابراز می کند. لذا پژوهش حاضر با هدف کلی بررسی تاثیر قرار گرفتن مادر در وضعیت های انتخابی خود بر شدت انواع دردهای مرحله اول زایمان انجام شد.
مواد و روش ها: در این کارآزمایی بالینی، 68 زن باردار که جنین تک قلو با عضو نمایش سر در سنین 38 تا 42 هفته و سیر طبیعی زایمان داشتند، در بیمارستان حضرت زینب (س) شهر مشهد در طی سال های 1383 و 1384 مورد بررسی قرار گرفتند. واحدهای پژوهش به طور تصادفی در گروه وضعیت های انتخابی و گروه وضعیت های متداول قرار گرفتند. طی بخش فعال مرحله اول زایمان، مادران در گروه وضعیت های انتخابی بسته به تمایل خودشان تغییر وضعیت می دادند، در حالی که مادران در گروه وضعیت های متداول، در بستر مراقبت می شدند. شدت انواع دردهای کمر و شکم هر یک ساعت به وسیله مقیاس تطابق دیداری ارزیابی می شد. جهت گردآوری داده ها از فرم های مصاحبه و مشاهده و مقیاس تطابق دیداری استفاده می شد. داده های حاصل با نرم افزار آماری SPSS و با استفاده از آزمون های تی دانشجویی، مجذور کای، من ویتنی و فیشر تجزیه و تحلیل شد.
یافته ها: نمره درد کلی (میانگین نمره های کمردرد و درد شکم) و نمره کمردرد در گروه وضعیت های انتخابی نسبت به گروه وضعیت های متداول به طور معنی داری کمتر بود (به ترتیب (P

Methods and materials: This clinical trial was carried out in 1383-84 (2004-5) at the labor ward of the Hazrate Zeinab (S) hospital in Mashad, Iran. Subjects were 68 low risk pregnant women with gestational age of 38 to 42 weeks, who were randomly assigned into two groups: cases (selective positioning) and controls (routine positioning). In the selective positioning group, mothers changed their positions at wish while mothers in the routine positioning group were given bedside care. Observation and interview checklists were used for data collection, and Student t-test, Chi-square, Mann-Whitney U and Fisher's exact test were used for data analysis in SPSS.
Results: Mean duration of the active phase of the first stage of the labor was significantly lower for the selective positioning group (p= 0.002). Mean ± standard deviation were 156.75±66.48 and 207.25±61.04 minutes for the selective and routine positioning groups respectively.
Conclusion: The study indicated that the duration of the active phase of the first stage of the labor was lower in the selective positioning group. Therefore, selective positioning can be advised for parturients to decrease the duration of staying mothers in maternity wards.
